Preparing the form does not create a request. Review the patient, contact number, clinic and time before confirmation.
Submission creates a real request; the clinic confirms by phone or WhatsApp. A 15-minute slot is an arrival time, not procedure duration.
Keep patient details and booking references private. Do not include them in URLs or static documents.
